Summer Reading & Math Requirements for rising 1st-8th graders are due on the first day of school and can be found on our website HERE.
First Day School Supply kits will be delivered to classrooms and available at the Meet the Teacher event for families who placed orders. For those who did not purchase a kit, school supply lists can be found HERE and can be brought to school on Meet the Teacher day.
All families should keep green carpool tags year-to-year. New families will receive carpool tags at one of our Back to School events. For replacement or additional tags, contact Susan Mackey at [email protected].
School uniforms for Junior Kindergarten - 8th grade students can be purchased in-store at Shaheens located at 994 Breckenridge Lane 40207, or at shaheens.com. Click HERE to view uniform requirements.

Health Records
Kindergarten & 6th Grade Students: All students entering Kindergarten, or students new to Kentucky, MUST have a complete physical within one year prior to entering that grade. Physician documentation of the physical, updated immunizations, and documentation of a Kentucky Eye Exam must be submitted to the school office prior to the start of the school year. Students entering sixth grade must have an updated immunization certificate and a sixth grade physical exam. Sports physicals cannot be accepted.
All students in Kindergarten through Eighth Grade must submit a current Certificate of Immunization Status to Saint Mary Academy.
